Observation

Ms Mempa submitted a Background check list form for annual compliance. It was not complete but it listed Gina i. Vessey as her sister and as a household member. Ms Vessey has not submitted a DCYF background check. It is dated 3/12/2022. The licensor has alerted Ms Mempa that this was missing on the form as was her own information. The form has not been returned yet and Ms Vessey has not submitted a DCYF background check yet at of 3/28/2022. The anniversary date, thus the annual compliance, is due on 4/03/2022. (no background check application 4/20/22 12:57 pm LR ) She said that Ms Vessey moved in " in October" 2021. 4/28/2022 Ms Mempa verified today that Ms Vessey does not have a TB test available for review and not within the last 12 months on file with her.
